Adoption Application Form

Neuse River Golden Retriever Rescue, Inc. (NRGRR) is a nonprofit organization with the mission of finding loving, “forever” families for homeless golden retrievers.  Our goldens come from a variety of backgrounds – saved from shelters, found as strays, or surrendered by their owners.

Goldens accepted into NRGRR receive medical care before being placed in foster homes for adoption. The adoption fee helps defray the medical expenses of the dog – which will include a physical exam by our vet, spay/neuter, shots/vaccines, a heartworm test and other medical treatments if needed.  Prospective adopters are screened to ensure goldens are placed in the best possible environment.  NRGRR adopters must sign a contract agreeing to various requirements, which includes a clause for the return of the adopted golden to NRGRR in the event the family is no longer able to care for the rescued dog. FAMILY

If you do not have small children living in your home, will your adopted dog come into regular contact with children such as grandchildren, neighbors, etc?
Does anyone in your household have allergies to animals?
Will this be your first dog as an individual/family (not including previous family pets as a child)?

ENVIRONMENT

Do you own or rent your home?
If you rent your home, do you have permission to have a dog over 50lbs?
Are you familiar with the use of a dog crate to train and/or confine the dog while you are not there?
If not, would you like to receive information on this training tool?
Are you planning to crate the dog?
Will your dog ever be kept in a basement or garage? If yes, please describe.
Will your dog ever be kept outside in a yard or outdoor kennel for extended periods of time?

EXERCISE AND CARE

Are you willing to pay for obedience school for your Golden?
Will the dog be walked daily?

FAMILIARITY WITH GOLDEN RETRIEVER BREED

Are you aware that golden retrievers are very active dogs as they were originally bred as a sporting/working dog?
Are you aware that they shed year round?
Are you aware that golden retrievers are large dogs and can unintentionally knock down children or the elderly?
Are you aware that routine costs of maintaining a dog, including vet care, food, heartworm preventative and flea/tick control average $800-$1000 per year?
Would you like additional literature on the golden retriever breed and their needs?

MEDICAL

Have you contacted your most recent veterinarian to authorize the release of information to NRGRR for the purposes of processing this application? If not, please do so before submitting this application so as to avoid a delay in processing.
Are you willing to pay for monthly heartworm preventative year round as well as other preventative meds and care?
